Description: Position Summary Staff Nurse II is accountable for the delivery of patient care utilizing the Nursing Process and Relationship-Based Care model as the framework. The Staff Nurse II provides direct patient care and takes initiative to improve patient outcomes by integrating Shared Leadership, Evidence Based Practice, Relationship Based Care and standards of nursing excellence into clinical practice. Through therapeutic relationships with patients, families, self and colleagues, the Staff Nurse II functions as an interdisciplinary team member who provides quality patient care and fulfills unit and organizational goals. Staff Nurse II facilitates educational development of staff, leads unit-based quality improvement initiatives and acts as a professional and clinical role model and educational resource for staff. Staff Nurse II is actively engaged in Shared Leadership through membership on a governing council. Duties & Responsibilities xe2x80xa2 Develops, implements, and updates the patientxe2x80x99s plan of care based on on-going assessment of the patient and input from the patientxe2x80x99s family._x000d_ xe2x80xa2 Shares information and collaborates with other members of the health care team to promote effective patient care. _x000d_ xe2x80xa2 Verifies and implements orders from physician, physician assistant and/or advance practice nurse by performing interventions as prescribed by the provider and documenting accurately in the electronic medical record. _x000d_ xe2x80xa2 Engages and obtains information from patient and/or family to develop an appropriate patient educational plan. _x000d_ xe2x80xa2 Recognizes symptoms, establishes priorities, and responds to patientxe2x80x99s changing needs. _x000d_ xe2x80xa2 Engages support personnel by delegating appropriate tasks per the Illinois State Nurse Practice Act and provides support and feedback. _x000d_ xe2x80xa2 Advocates for patients, families, and their significant others, the community, and the nursing profession. _x000d_ xe2x80xa2 Assumes role of Charge Nurse when assigned, utilizing appropriate resources and tools. _x000d_ xe2x80xa2 Integrates theoretical knowledge, evidence-based research, current standards of practice and clinical experience to provide xe2x80x9cqualityxe2x80x9d patient care as measured by established nursing quality standards and other governing/accrediting health care bodies. _x000d_ xe2x80xa2 Takes responsibility for continued professional growth/development by identification of learning needs and initiates strategies to meet such needs. _x000d_ xe2x80xa2 Identifies recurring clinical practice and/or related unit problems and participates in problem solving and resolution. _x000d_ xe2x80xa2 Plans, presents, and evaluates learning/educational experiences for staff and students. _x000d_ xe2x80xa2 Serves as preceptor and/or resource to other nursing, support staff and students. _x000d_ xe2x80xa2 In collaboration with Director/ANIII/CNCII leads unit-based quality initiatives and provides feedback and education to staff. Qualifications: Minimum Qualifications Licensure as a Registered Professional Nurse in Illinois with the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation. One year of RN experience. Current AHA BLS/CPR and other required unit specific certifications. Effective communication skills (oral and written) that facilitate therapeutic relationships between patients, family, and health care providers. Intermediate computer skills with the ability to become proficient in the Electronic Health Record (EHR) with on-the-job training. Excellent attendance, interpersonal skills and work history required. A minimum of five years of current hemodialysis experience is required. Home dialysis experience, CNN, or CDN certification is preferred. Effective communication skills (oral and written) that facilitate therapeutic relationships between patients, families, and health care providers. Is self-directed and works well in groups/teams exercising sound clinical judgment Intermediate computer skills with the ability to become proficient in the Electronic Health Record (EHR) with on-the-job training Additional Required Qualifications: Two years of current experience working with inpatient and outpatient dialysis settings required. Familiar with Hemodialysis workflow and regularity standards required. Knowledge and experience working with CKD and AKI patients, renal replacement treatment Peritoneal dialysis, Home Hemodialysis, Acute inpatient Dialysis and Incenter Chronic Hemodialysis.
