Responsibilities:- Works in collaboration with physician in formulating treatment plans for health problems and follow-up- Conduct comprehensive health assessments and physical examinations- Diagnose and treat chronic illnesses- Develop and implement treatment plans, including medication management- Provide patient education on disease prevention, health promotion, and medication adherence- Utilize electronic medical record systems (e.g., Epic, Medent) for documentation and communication – Performs other related duties as required Requirements:- Valid Nurse Practitioner license in [Ohio]- Master’s degree in Nursing with specialization as a Nurse Practitioner- Certification as a Nurse Practitioner in [Family / Adult]- Proficiency in performing physical examinations, triage, and utilization review- Experience with medication administration and phlebotomy- Knowledge of laboratory tests and interpretation of results- Ability to independently diagnose and manage common acute and chronic conditions- Strong communication skills to effectively interact with patients, families, and healthcare team members Note: This job description is not intended to be all-inclusive.
